Forms allow standardized data collection for inspections, checklists, and procedures.
Understanding Forms
Creating a Form Template (Gateway)
- Navigate to Logbook > Forms > Templates
- Click Create Template
- Enter:
- Form Name: e.g., "Daily Pump Inspection"
- Description: Purpose and instructions
- Equipment Type: Which equipment uses this form
- Add fields:
Field Types
Building the Form
- Click Add Field
- Select field type
- Configure:
- Label
- Required (yes/no)
- Help text
- Validation rules
- Drag to reorder fields
- Save template
Example: Pump Inspection Form
Form: Daily Pump Inspection

Assigning Forms to Equipment
- Open form template
- Click Assign to Equipment
- Select equipment items or types
- Set schedule (optional):
- Daily, Weekly, Monthly
- Specific days/times
- Save assignments
Reviewing Form Submissions
- Navigate to Logbook > Forms > Submissions
- Filter by:
- Form template
- Equipment
- Date range
- Submitter
- View submission details
- Export for reporting
